OBEYAH VIRTUAL (ZOOM) GROUP AND CHAT AGREEMENT

As the parent or legal guardian of the student indicated on the form, I grant permission for ObeYah Youth Ministry to engage my child in the use of video and audio conferencing with their ObeYah Youth Ministry groups. OBEYAH TEXT/CHAT AGREEMENT

ObeYah Texting App Agreement As the parent or legal guardian of the youth indicated on the form, I grant permission for ObeYah Youth Ministry to engage my student through App messaging to their group leaders. These text messages will be for the purpose of sharing group content. One or more of the following apps will be used: WhatsApp Viber Telegram Signal Wickr Me Facebook Messenger Tox GroupMe My minor has permission to receive text messages from ObeYah Youth Ministry Leaders. Parental Consent Participation & Liability Release

The undersigned does hereby give permission for my child (“Participant”), to attend and participate in any ObeYah youth ministry activities, events, retreats. LIABILITY RELEASE: In consideration of ObeYah allowing the Participant to participate in children/youth ministry (Sabbath gathering, online Meetings, Activities, Events, Retreats, Lock-Ins, Trips) and childcare, I, the undersigned, do hereby release, forever discharge and agree to hold harmless ObeYah, its Morehs, directors, employees, volunteers and teachers (collectively herein the “Great Awakening”) from any and all liability, claims or demands for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the Participant while involved in the children/youth activities and childcare. I the parent or legal guardian of this Participant hereby grant my permission for the Participant to participate fully in children/youth ministry activities and child care, including trips. Furthermore, I, on behalf of my minor Participant, hereby assume all risk of accidental personal injury, sickness, death, damage and expense as a result of participation in recreation and work activities involved therein. The undersigned further hereby agrees to hold harmless and indemnify Great Awakening for any liability sustained by the Great Awakening as the result of the negligent, willful or intentional acts of said Participant, including expenses incurred attendant thereto.
